Depending on your current health insurance provider or employee benefit plan, it is possible for services to be covered in full or in part. Once you have scheduled your first appointment our staff will request a copy of your insurance card to be sent to email@example.com. Our staff will use this copy to call and verify your insurance benefits and eligibility before the time of your initial appointment. Our office does not accept Medicare or Medicaid.
Self Pay Rate
If you do not have insurance or if you have an insurance not accepted by our office we do offer a self pay rate for services. Self pay rates vary depending on which services you are receiving.
We accept cash, check, and most major credit cards as forms of payment.
If you are unable to attend a session, please make sure to call and cancel at least 24 hours beforehand. Otherwise, you may be charged a late cancellation, rescheduling, or no-show fee for the session.
